What is Support Coordination?

Support Coordination is like having a helpful guide on your journey with the National Disability Insurance Scheme (NDIS). It's all about making the most of your supports and reaching your goals. A Support Coordinator’s role is to help you understand use Your NDIS Plan & how to use it to achieve your goals, connecting with services, NDIS providers, community groups, and government services that can support you along the way. Support Coordinators aim to build skills and confidence so you can coordinate your supports more effectively.

There are three levels of support coordination available:

Support Connection;

This helps you connect with different types of supports, whether they're informal, community-based, or funded.

Support Coordination;

This level helps you understand and use your NDIS plan better. You will work together to make sure you're getting the right mix of supports to help you live more independently and be part of your community through your NDIS, community and other government services.

Specialist Support Coordination;

If your situation is more complex, this level offers specialised support. A Specialist Support Coordinators requires specific experience and qualifications to help you manage challenges in your support environment and ensure you receive consistent service.

We'll tailor our support to meet your unique needs and help you succeed.
